Step 1: Apply online
You will need an email address to apply for E3. If you need to, you can create a free email account using Google, Outlook, or Yahoo. Be sure to check your email often for updates! Step 2: Submit your paperwork to enroll
This step is done by email and invitation only. Enrollment is the opportunity to complete and sign required paperwork and submit copies or pictures of documents necessary to continue beyond the apply step. You will complete enrollment forms on a computer or a mobile device and using a secure folder (ShareFile) submit your forms and vital documents. Details will be sent to the email address used on your application.

Gather your docs! You must provide proof of the following to receive services:

Continue to check your emails daily for updates on your status of your file forms and documents. Check your junk or spam email folders just in case the emails do not get sent to your inbox.
Step 3: CUSTOMIZE YOUR SERVICES
Once you have been confirmed as eligible, your provider will reach out to schedule your assessment and goal planning session to determine the best services for you.
